how to decide whether choose Engineering or medical as career?

I am confused between engineering and medical. Which stream is better?

Both are best field among many others . You have to think in which you are best to take career path. If you are good in bio go with medical but if you are good in maths ,means your analytical and calculation capabilities are good which requires in technical field , then choose Engineering.
